We are a Community Mental Health Team, we see people aged between 16 and 65 registered with 3 local G.P. practices. We help people with mental health issues that would be considered to be either moderate or severe. Examples of this could be someone experiencing psychotic symptoms or severe depression. The Community Mental Health Team consists of community psychiatric nurses, psychiatric social worker, psychiatrist, registrar occupational therapist and carer development worker. We see people in the community or at an agreed venue. We try to establish a trusting relationship with the person referred so that together we can try to understand the mental health difficulties and then agree a plan of how best to help.
